Tirzepatide dose escalation

Mounjaro dosage guide

Mounjaro commonly starts at 2.5 mg once weekly, increases to 5 mg after the introductory period and may then rise in 2.5 mg steps when clinically appropriate. These stages are not an automatic instruction to increase: the prescriber reviews progress, symptoms, side effects and tolerability before changing the dose.

2.5 mgThe usual introductory weekly dose; it is the beginning of assessment, not a dose to rush through.
2.5 mg stepsFurther increases are gradual and considered only after the current dose is reviewed.
15 mg maximumThe highest authorised weekly dose—not a target that every patient needs or tolerates.
Usual escalation pathway

Start low, review, and increase only when appropriate

The chart shows the available staged sequence. Some patients stay on 5 mg, 7.5 mg, 10 mg or another prescribed strength for longer, and a dose should not increase while important side effects or poor intake are unresolved.

The timetable is a general guide. Your prescription, the current product leaflet and the doctor’s individualized plan take priority.
1

Assess response

Review appetite, weight trend, nutrition, hydration, glucose risk and clinical goals.

2

Review tolerability

Discuss nausea, vomiting, constipation, diarrhoea, reflux or abdominal pain.

3

Choose the next step

Continue, delay an increase, investigate symptoms or change the plan with the prescriber.

01

The usual escalation sequence

The available sequence is 2.5 mg, 5 mg, 7.5 mg, 10 mg, 12.5 mg and 15 mg once weekly. One month before a potential increase is a usual minimum stage, not a deadline, and not every patient needs the highest dose.

02

Review before moving up

The doctor considers appetite, weight trend, digestive symptoms, hydration, other medicines and glucose risk. An increase should not proceed merely because a calendar date has arrived.

03

If the pen or prescription looks different

Spanish presentations may differ from products seen online or supplied elsewhere. Check the exact label, concentration and instructions; do not transfer internet dosing advice between presentations.

!
Safety first

Use only the exact prescribed presentation

Never combine strengths, reuse needles, share a pen, estimate an unmarked dose or calculate a “Wegovy equivalent.” Contact the prescriber or pharmacist if anything is unclear.

Common questions

Mounjaro dosage guide: questions

Do I have to reach 15 mg?

No. The effective and tolerated maintenance dose differs between patients. The highest authorised dose is not a target everyone must reach.

Can I increase Mounjaro faster?

No. Increases should not be rushed or improvised. The prescriber reviews tolerability and clinical response before changing strength.

What if 5 mg is working?

The doctor may decide to continue a tolerated effective dose rather than increase automatically.

What if I have side effects before an increase?

Contact the prescriber before increasing. Persistent digestive symptoms, dehydration or poor intake may require a delay, review or different plan.

Can I remain on one dose for longer?

Sometimes. A clinician may keep a patient at a tolerated dose for longer when response, symptoms or another clinical factor makes that appropriate.

What if I miss a Mounjaro dose?

Follow the current product leaflet and contact the prescriber when timing is uncertain. Do not take extra medicine to compensate.

Can I restart after missing several weeks?

Ask the prescriber. The safest restart may differ from your previous dose depending on the length of the break and previous tolerability.

Can I switch directly to Wegovy?

Do not switch or calculate an equivalent dose yourself. The clinician must review the last injection, indication, side effects and new starting plan.
